Some customers have experienced an error when trying to download the forms. If for any reason you have problems, please contact us and we'll email them directly to you.

For our customers that are considering moderate to significant upgrades to there trucks and would like to discuss their plan, please fill out the Customer Worksheet and email it to rich@doghousediesel.com.

There is no need to print it, just fill it out and email it in. Once we get it and review it, we'll email you back and let you know we've seen it and you can give us a call at your leisure

This allows us an opportunity to get you some information and options before calling; this saves us both time and ultimately gets you to your end goal faster.
